Most likely: Work of breathing, i.e., the energy spent to breath in people with COPD is high. On top of this, your cardiomyopathy does not help to push blood through and oxygenize then sensed by your body to even try harder. You may just need a medicine to decrease the sensation of dyspnea and you may feel better. Talk to your lung specialist.

Complicated question: Could be related to copd, to cardiomyopathy or to another condition or medications ( I do not see any meds for htn). You really should see a doc face to face to have a few simple things evaluated. Consider oxygenation, sleep apnea, atrial fibrillation, worsening cm, thyroid, anemia, etc.
