Degree: The difference is all in the degree of the injury. In a sprained ankle, some of the ligaments that support the ankle are stretched or even torn, but the ankle bone remains within the ankle joint. A dislocation is a much more severe injury in which the ankle bone comes completely out of the joint. It is horribly painful and considered an emergency to be reduced immediately, followed by casting.

See below: You would not be able to walk or stand on a dislocated ankle and you would be in terrible pain. Dislocation would usually be a trip to the or to try to relocate the ankle under anesthesia.
