There is never: 100% accuracy. It is very accurate. You can repeat in a week using first morning void if you still have not had your period.

It is possible: Without knowing any other information it is hard to make a good estimation of what the probability is, however it is not uncommon for a woman's cycle to be off by a few days in any given month. I suppose the important questions are: Are you sexually active and do you use any birth control or condoms?
