Depends: There are several factors. A general answer is about weight 7-8. The weight of the patient is a big factor. If the patient is obese the abdominal sonogram will be challenging. The skill of the technician is another. The age of the sono machine is another.

Depends: For routine fetal evaluation in a normal size patient, we might do an abdominal exam after 8 weeks. For an obese patient we might do a vaginal exam until 12 weeks. For evaluation of a low placenta we frequently do a vaginal exam in the 3rd trimester. It just depends.
