Hhhhmmm: This answer could probably fill a thousand page book and at the risk of being too simplistic i will say in my opinion the majority of cardiomyopathy are caused by extrinsic factors such as viral infection and this is subject to the health of the population at risk and so socioeconomic factors play stronger meaning low se groups are at higher risk and this may appear to be of a racial nature.
