Congenital obesity: Congenital abnormalities like patent ductus arteriosus, tetralogy of fallout, patent foramen ovale. Valve issues. Occasionally heat prostration and electrolyte issues (2 a day football practices in august). Childhood morbid obesity, endocrine issues.

In Addition: To the above, there can rarely be origin of the coronary arteries from the pulmonary artery rather than the aorta. As a result, the heart is supplied by blood with low oxygen concentration resulting in heart attacks at an early age.
