The causes of isolated right heart failure include primary pulmonary hypertension and pulmonary diseases associated with pulmonary hypertension. Left to right cardiac shunts cause right heart failure and mitral stenosis is another(techinically this is a left heart cause.

dullness and heaviness- like discomfort of Right upper quadrant noticed by patient, liver is tender and enlarged , noticed by the doctor. causes are many, but could be relatet as a complication of Lt. heart failure which induce congested lung fields and in consequence, increased pulmonary vasculare resistence that oppose the rt heart pump mechanism. long term consequence is Rt heart failure.

Physical exam: Fluid accumulates during congestive heart failure. When the right side of the heart fails, fluid accumulates in the legs and abdomen. When the left side of the heart fails, fluid accumulates in the lungs and makes the patient short of breath. Not infrequently, both sides of the heart fail at the same time.
