Yes definitely: In fact you can have pcos without having multiple small cysts on the ovary. The main issues with pcos are absent or infrequent periods, acne or abnormal hair growth and Insulin resistance or obesity. The appearance of the ovary on ultrasound is a reflection of the hormonal imbalances going on in the body with pcos.
