Dermatologist helps: A primary care doctor can check to see what the patches are all about. If needed, a dermatologist can help also, especially if some skin lightening creams might be used. If there is something causing the darkening, such as acne spots that turn dark, or dry sensitive eczema patches that turn dark, or even tinea "ringworm" that turns dark, then the doctor will first treat that primary disorder.
