diagnostic tests: A blood clot is usually diagnosed through ultrasound and blood tests. Sciatica can be diagnosed by an MRI of the lumbar spine. Speak to your doctor and they can determine the appropriate tests to run.

Generally you can't: Unless you have a fever or swollen limb which may suggest a clot more than sciatica otherwise not very specific. I would suggest ruling out the clot first and then workup the sciatica issue afterwards.
