A needle: Is a needle and uncomfortable. Have the doc lay you down, ask the doc to count to three and cough when he says two....And three, you will feel it less.....

Depends: If your doctor is rushed and shoves the needle and injects the anesthetic quickly then yes, it will hurt real bad. If your doctor is patient and injects slowly then often you will only feel a small pinch.
