See your doctor.: Those are strong medications.  It is best to follow closely with your physician on this one.  Doing it on your own, and doing it wrong could cause harmful side effects like seizures.  I know you are asking for a safe titration schedule, but due to the nature of these medications I would personally recommend not do this on your own.

no: I don't know if you mean once, by mistake, or regularly.  If you doubled your dose by mistake once, it's unlikely to be harmful, although you might suffer serotonin side-effects (stomach upset, jitteriness, restlessness).  Taking 300 on a regular basis usually does more harm than good.  It's not okay if your doctor didn't prescribe it that way.

I find that it works for a while at one dose, and that the dose may need to be increased several times during treatment because of depression breakthrough. Sometimes it stops working altogether and then this is the tie that your psychiatrist will really earn his or her money coming up with a new treatment regimen which may still include zoloft (sertraline)..

Difficult.: Patients are usually on these medications for anxiety/ depression symptoms.  It does alter the brain chemistry, so to allow the body to slowly adapt to not having it is tricky and can take months to years.  You could feel a rebound of the original symptoms and/or other effects if you stop too quick.
