Likely a varicocele,: nearly always of left testicle. It can sometimes cause pain &amp; may contribute to reduced sperm production/fertility due to associated venous congestion &amp; impeding capillary function within the testes. If you choose to treat/stop, best option is careful embolization of left renal veins via a catheter (removes head pressure &amp; blood returns via right veins); much better than older surgical methods.

Epididymis swelling: Could explain lump. This would be benign. Check to see if present without masturbating. Epididymal cysts usuaally do not require surgery unless patient thinks beefits of surgical removal are worth the trouble.

Probably Normal: Sounds like you are feeling tiny lymph glans surrounding the vas deferens  (the tube that carries the sperm from the testicles to your seminal vesicles A good question to ask is "does it feel the same on both sides??  If YES its probably normal..if NO see your Primary Care Doctor.

Need more info: Need more info. Do you feel one small testis on the left?  Do you feel a normal testis and an additional small "ball"?  If two "balls" on left side are they connected or do they feel separate?  This could be just a small testis, spermatocele, epididymal cyst, varicocele, or an abnormal growth.  Either way see your urologist and have an examination and likely a scrotal ultrasound to sort this out.

Scrotum: Probably not anything serious.  Some testes are hyper mobile and will pull up into into the inguinal region.   I would recommend an exam by a urologist in regard to the size discrepancy. (is one unusually large or is one unusually small?).

Tolnaftate: A tolnaftate cream is still probably best for most athlete's foot, a diagnosis a lay person can make easily.  I'd apply it to the sensitive area on your bottom as well and see what happens.  Keep these places dry especially after you exercise.  If you don't get a good result in a week, physician's office visit is in order.

Abscess: Sounds like a small abscess (boil). If it doesn't get bigger and there is no pain or redness, it will probly resolve on its own since the pus is draining. Apply warm compresses. See doctor if worse.
