Not usually: caffeine however is a stimulant, and can cause anxiety and the rate of breathing may go up - breathing faster and more deeply. This may result in low carbon dioxide levels, which mimic feeling short of breath. Cut WAY back in your caffeine use if this is happening to you. You may need to do a gradual taper, because rapid cessation may cause severe headaches and other withdrawal symptoms.

See doctor : If your shortness of breath is truly from anemia then that means your blood level is pretty low, and you should be evaluated by a doctor and you might need a blood transfusion. Shortness of breath could be due to other reasons even if u have anemia. Anxiety, fast heart rate (from caffeine ) etc.

Yes and no: Fibrocystic breasts can cause pain and that pain can be worsened by caffeine intake.  Nevertheless, it is not normally associated with shortness of breath.  I advise you to seek prompt medical attention.

A suggestion: Do an ekg at the time that this happens. Also do a spirometry and pulse oximetry. Make sure these tests are performed when you are short of breath. Most internists can do them in their office.
