Tiniaversicolor: There is a type of very mild fungal skin infection called tinia versicolor. It causes patchy loss of pigment mostly on the trunk. It may be slightly flaky. It is easily treated with topical treatment such as selsun blue. Also topical and oral anti-fungal treatment. Your pcp should be able to treat this.

Hyperpigmentation : Hyperpigmentation is very common in dark skin.  It can occur after a rash, acne, a chemical peel, or laser resurfacing.  It depends a lot on how aggressive the procedure is, as well.  I would carefully discuss with your dermatologist prior to a procedure.  Perhaps a test area prior to scheduling the full treatment would be prudent to see if you tend to develop hyperpigmentation. I hope that helps!

Not natural: You have melasma, which is caused by the sun, hormones ( including the bcp) and other reasons that we do not understand. It is usually treated with bleaching creams and the constant use of a sunblock which has titanium dioxide or zinc oxide in it.

Pigmentation and CRF: The adrenal glands are positioned directly above the kidneys.  As the kidneys are failing, the adrenal glands are also often affected.  As the brain tries to signal for increased adrenal activity you get more adrenocorticotropic hormone (acth), this can cause increased melanocyte stimulating hormone activity causing the increasd pigmentation associated with chronic renal failure.

Hydroquinone: 4% Hydroquinone is the one we usually prescribed. Tri-Luma cream is the most popular. The efficacy depends individually.
Metasone ointment is a relatively high potent cortisone ointment, not safe for areas above neck, skin folds, genitalia.

Skin indentation: Was it after trauma or  pressure  on skin? Skin changes  that  do not resolve should  be evaluated. Where was the skin indentation?  if on the breast mammogram  with  sonogram is  needed .  If on other parts of skin face, or arms / legs  may be early signs of basal cell cancer. Follow up with dermatologist.

Combination Therapy!: 1. Broad spectrum sunblock daily 2. Serial superficial chemical peels, performed by an esthetician or a physician 3. Prescription creams like a retinoid (tretinoin) or fade cream (hydroquinone) 4. Laser treatments by a physician.
