YES: It just takes one sperm to get pregnant.  
The bigger question is why does she do that?  Condoms are used to prevent the exchange of bodily fluids.  Putting the fuids in her vagina after sex is no different than not using a condom at all.

Sperm Issue: If his morphology is at 0%, it would be difficult to get pregnant spontaneously, but not impossible. Your partner may have a problem with his sperm cord or another urologic problem.  The best advice would be for him to see a urologist and/or repeat the semen analysis.

Probably not: Take a test.  It depends on your ovulatory cycle but an IUD prevents implantation so even if you had conceived it would not implant.  Removing the IUD after a week?  Still unlikely you would get pregnant but it is theoretically possible.

Depends: Sperm can reach egg already in fallopian tube in 30 - 60 minutes. However sperm can survive a few days in the fallopian tube until an ovum (egg) enters the tube ; before they both meet.
