Maybe aphasia: I am glad you had your hearing checked first as that would be the initial thing to look into. If those tests are normal, then how your brain processes speech. Some strokes can affect the part of your brain that allows you to hear and speak properly. Problems are called aphasias. Also, even with normal hearing, some people can be distracted if trying to hear with lots going on. Get checked out.

Likely distractions: Even with normal hearing, some people can find hearing in noisy places makes it tough. Distractions can be a problem for some people, and their brains may not be able to filter one thing out from the rest. And if your hearing is on the low end of normal, or if it has decreased from your normal, you may have relative hearing loss as this problem is common as hearing decreases.

i have difficulty understanding speech in the background, even with my hearing aid, is there a way to make it better?

Digital/programable: I wear hearing aids myself and have experienced the same issue. I have found that a digital/programable hearing aid with the ability to adjust it for the environment you're in works the best. Nothing is perfect, but this option is pretty close!

Need to examine: It is not feasible to provide a meaningful opinion without taking additional history, physical examination and may be some tests. It would be prudent to see your doctor and discuss if you may be depressed. Counter-intuitive as it may seem, exercise may help. Start with low intensity exercise such as walking and gradually increase the intensity as tolerated.
Wish you good health! - Have a diet rich in fresh vegetables, fruits, whole grains, milk and milk products, nuts, beans, legumes, lentils and small amounts of lean meats. Avoid saturated fats. Drink enough water daily, so that your urine is mostly colorless. Exercise at least 150 minutes/week and increase the intensity of exercise gradually. Do not use tobacco, alcohol, weed or street drugs in any form. 
Practice safe sex, if you have sex. 
Get HPV vaccine

should i be concerned that strangers can't understand my child's speech?

Clarity takes time: Half of what a 2yo says should be understood by strangers, 3/4 by 3 & 95% by 4. Local school districts begin looking for those with speech or language issues starting at 3 yo so the can be brought up to peer level by school entry. Share any concerns with your pediatrician and see if a referral is appropriate.

Comes in stages: Most kids will emerge as copycat talkers, trying to mimic what they hear. Clarity comes with repitition, gental redirection and eventual confidence. About half what they say at 2 should be understood by strangers(3/4 by 3 and 95% by 4). Any kid that doesn't should be screened. School districts operate child find programs to get these caught up by school entry.

Most likely, yes.: A Speech/Language Therapist will evaluate you, then discuss therapy &amp; prognosis. Is  your Articulation Disorder Developmental, from childhood, or from a Central Nervous System insult, Neuromuscular Disorder, Hearing Impairment or any other medical issue? If the onset was in adulthood, I trust you've seen your primary physician in order to find the etiology, or cause. (S)he can refer you to an SLT.

Could be bad.: I'm not sure how old you are, but I agree that your worst case scenario here is a stroke, which of course could be very bad.  You should be evaluated in an ED right away.

Can't hurt: How do you know that many times people can't understand you if you can't tell? If you have any questions about your ability to speak clearly & enunciate, i think it's perfectly reasonable to get a professional evaluation by someone trained to assess speech & communication. Just be prepared to pay out of pocket in case your insurance won't.
