Lipid effect, flush: Niacin is a b vitamin that is often used to treat lipid (cholesterol and triglyceride) disorders. Nicotinamide is another vitamin in the vitamin b group. Although Niacin and nicotinamide are identical in their vitamin activity, nicotinamide has no significant cholesterol or triglyceride effects.  Also, nicotinamide does not cause skin flushihg that can be a limitation to use of  niacin.

Rate of action: niaspan is extended release, where as niacin otc is not. Some literature suggests risk of liver toxicity is lower with the extended release versions. I believe it's possible to obtain otc nicotinic acid. Ask the store

Hx noted: OC Niacin is niacin. Generic OC niacin is likely cheaper and thus preferred.  The usual dose is 500-2000 mg limited by flushing. Flushing can be reduced by taking 325 mg of aspirin an hour beforehand. Niacin reduces triglycerides but generally isn't indicated unless the level is &gt;500 and refractory to dietary changes.

Long acting: Niacin is useful to lower cholesterol level and has some other uses.  Taken as a standard pill, it commonly causes an itchy flushing starting about 1/2 hour after swallowing.  "flush free" is formulated usually with delayed release so these side effects are absent or minimized.  This does not affect the potency.

No: Just uncomfortable. The mechanism of the flush (vasodilation) is the same mechanism that occurs when you're embarrassed and your face turns red and you feel hot. Uncomfortable, but not dangerous.

Discuss with MD: Yes but Flush free has less side effects.
 Niacin  is no good for lowering bad cholesterol. It may improve (a little) the good cholesterol and the best effect is on triglycerides. BUT, newer studies show NO benefits in mortality rates or frequency of heart attacks or strokes. Indication of Niacin has become more questionable since you may improve "numbers" but you do NOT improve outcomes.

Clearing what?: Niacin is only prescribed for high triglycerides and sometimes for cardiac patients with other lipid abnormalities. It's not a general body cleanser and why would you expect such a thing?
