Infections &  hormonal changes can cause spotting. If there is a concern for pregnancy then do  a home pregnancy test .  There are many options available to help regulate the menstrual cycle. Schedule a visit.

No ovulation?: It sounds like you either didn't ovulate or you could have a cyst. 

Are your cycles regular? It's near impossible to not get a period if you ovulated unless you have a cyst that is producing hormone that is prevent your cycle. Sometimes this can occur on medications like Clomid (clomiphene).

I would see your Ob/Gyn if you don't get your period in the next week and your hpt are still negative.

Pregnant?: Pregnancy is always a possibility.  Purchase and perform a home pregnancy test.  They usually become positive when you're about a week late.  If it's negative, repeat it in 1 week and see your doctor.

Period starting? : Brown discharge is just oxidized blood, and periods are rarely precisely regular (they may vary 1-3 days and still be "regular").  If your normal flow does not start, wait a week and obtain a serum HCG test to be certain about pregnancy.

Pap smear ?'s: With "no neoplasia", a "reactive pap smear" refers only to inflammatory changes in the cell appearance, and is not a cause for concern.

Old blood/tissue: During your period, the lining of your uterus is shed and expelled through your vagina as red blood and tissue.  At times, some blood is retained in the vagina and turns brown, before leaking out.  It is nothing to worry about.
