Adrenal Hemorrhage: Not certain what you mean by "back passage".  If they operated on the adrenal and have a drain in place one might see bleeding.  They adrenal has been put in a hard to get and see region, prob for our protection. Check with your Dr.   I would do so now.

Tiredness: There are many causes of your symptoms.  A failure of your adrenals, Addison's disease, causes fatigue, but this is rare, and unlikely as a cause.  The term "Adrenal Fatigue" is popular on the internet, but is mostly a myth. Adrenals either work, or don't work, and you would need an ACTH test to figure that out.

It is often: Difficult to determine the specific cause, but a  good endocrinologist can usually figure it out with history, physical, lab, x-rays etc.

Many causes: A severed artery or big vein, a ruptured organ most notably the liver, an ulcer or cancer causing bleeding into the gut, lack of platelets causing bleeding from the rectum, esopageal varices from liver disease that rupture and cause fatal vomiting or passage of blood -- all these and many others. The dying person feels primarily a sensation of cold; if you've lost someone you have my sympathy.

Depends on : amount of bleeding, but make an appointment with your doctor, and  let them know how heavy you  are bleeding, so they can help with timing of appt. Yes, you should be seen to evaluate this bleeding.

Firm pressure.: Simply applying firm pressure to the site of bleeding will almost always cause it to stop. Direct pressure slows the flow of blood and allows time for clot formation.

Bleeding : I assume you have vaginal bleeding. You should see a doctor. Go to the emergency room if you have dizziness, chest pain or fatigue. There are several possible reasons for this.
