Lots can happen: It depends on what kind of hernia it is. Nothing can happen or you can get an organ or part of an organ stuck in it. Twisting in the hernia can cut off blood supply and cause gangrene. I believe in "fix 'em when you find 'em". Others say if it doesn't bother you, leave it alone. You will have to decide with the help of your doctor.

Same or worse: They don't go away, or better by themselves. May not progress.
Elective surgery for hernias (inguinal/groin, femoral, abd wall-incisional, spigelian) that cause symptoms.
If hernia gets bigger,  gut can get stuck (incarcerated): urgent surgical repair.  If  gut gets stuck and blood supply compromised ("strangulated"):surgical emergency.  Usually younger is better surg risk vs older. Truss nonsurgic.

Hernias: Will enlarge and become painful or cosmetically unacceptable.  If it becomes painful, then it can incarcerate to strangulate, so it becomes urgent that you undergo surgical repair.  Show it to a surgeon for an opinion on when it needs to be repaired.

Hernia: Any type of straining can increase the symptoms from a preexisting hernia. This type of activity is unlikely to cause a hernia but may unmask a small previously asymptomatic hernia.

NO if no weakness: Excessive strain or cough may result a hernia if there is weakness in inguinal floor, in a normal person with out weakness , with normal strong inguinal floor , will not cause hernia by excessive cough.

Yes or no: If you have potential weak area at hernial site, or a  small hernia you did not notice ( to be pointed out by your physician ), or a weak scar from previous surgery you  may develop hernia. If you do not have those , not much chance of getting a hernia, go for abdominal work out.
