Post op changes: The radiologist is describing the part of your lateral meniscus (cartilage pad in the middle of your knee on the outside half of the joint) that looks different than it usually does, like it was cut or shaved down or there was a torn rim of the cartilage that was removed.  Your ortho surgeon should be able to confirm that is what he did in the surgery.

Osteoarthritis: This is typically seen with osteoarthritis.  As the joint space narrows the meniscus can be extruded or pushed out of the joint space.  You should be evaluated by an orthopedist if you are having pain.  There are several treatment options including physical therapy, nsaid's, and injections.

Not necessary.......: It will depend upon your lifestyle and how active are you. If not highly active, try conservative treatment like pain management with medication, steroid injection for inflammation and physical therapy for quadricep strengthening. If conservative treatment fails, then reevaluation is needed.

menisci: linear oblique truncation of the posterior inferior most aspect of the posterior horn medial menisci, consistent with a tear. 
what does this mean, is this a serious tear? no symptoms?

Probably not: Symptoms guide the treatment of inevitable tears in the meniscus, a cartilaginous pad between the two long bones of the leg. Nerves that sense joint angle, position in space, and trauma are in cartilage. If there is a tear that creates an otherwise harmless flap that catches or folds back with movement there can be an amplified sensation like joint lock-up. If it don't hurt, don't fix it
