chest tightness/discomfort and chest pain,difficult in breathing and somehow i feel dizzy too. what does it means those symptoms?

Chest pain: You are under 40 so  cardiac causes for chest pain unusual. However, if you do substances like cocaine or meth that can be cause for chest pain. More common causes would be some type of asthma or anxiety. Fever? lower respiratory infection  possible. I do not know your other risk factors, but PE on list too!

chest tightness sob chest pain for two months. clear chest ct, 3cxr,3ekg,pft good, lots of blood work. albuterol mask at er, and rescue inhaler do nothing. only thing that seems to work is ativan, (lorazepam) and it works well. does this mean its anxiety?

Anxiety: Anxiety is capable of causing your symptoms especially if they are more noticeable at rest than during exertion and in view of your negative tests and response to ativan (lorazepam). For completeness a treadmill stress test and echocardiogram can be done,but I recommend that you discuss stress management with your physician and avoid alcohol,caffeine and nicotine.

Panic Attack: This is anxiety. Pure and  simple.  You are not having a heart attack. What is causing you stress? Breath!

GERD: Have you had a workup of your esophagus? Acid reflux is the commonest cause of severe, on-going chest pain. Has it been ruled out?

Yes: Yes, anxiety can present as palpitations and chest pain, however stress can also provoke a heart attack. Please go to ED and get checked out that this is not true chest pain.  Heartburn can also sometimes mimic chest pain HOWEVER, those are all secondary diagnoses AFTER we make sure it is not a heart attack...SEE A DR NOW!

i had severe chest pain/tightness during flight takeoff which lasted until we leveled off. what could this be and what should i do before flying again?

Breath: During these times, slowly take a deep breath in to fully expand your ribs/chest.  Then slowly exhale.  Repeat until level flight.
