Pinched nerve: Because patients having cardiac problems can have these symptoms, you should have this ruled out.  But the most common cause is an irritation of a nerve in the neck by a bone spur or herniation that can cause pain or numbness that runs down the arm into the hand.  An MRI can usually show the cause of the problem if it does not resolve.  Thank you for the question.

Doctor can evaluate: Chest tightness or shortness of breath is not normal, unless it is the mild temporary shortness of breath with sports activities (but even then, one must be sure he is not having more shortness of breath than his sports peers have). Possible reasons for shortness of breath include lung or heart disease, fluid around the heart or in the lungs, asthma, anxiety, costochondritis, etc...

If you are diabetic your doctor should be informed. The sensation in your arm could be from a long list of conditions. If it gets worse or paralysis develops seek urgent care.

Depending on what part of the hand is involved depends on which nerve it is. The best thing to do is see you doctor and he can order tests called Nerve Conduction tests that can see where the nerve is pinched and determine treatment from there.
