Constipation: Most likely gas is trapped and reaching a sensitive spot inside your intestines when you lie down.  Change positions, best to sleep on stomach so you can easily pass gas.  Plenty of fluids, fiber, fruits, and keep yourself regular to keep from trapping gas.  Good luck!

Epigastric pain: He has a BMI of 49 and this is a risk for GERD.  Other possibilities are gastritis, ulcer, gallbladder problem.  I would also be concerned about a possible cardiac problem.  I would get checked.

Get eval: 2 years is along time to be dealing with a burning pain in your abdominal area.  See a dr and get some testing to determine what is causing your sensations.  Diverticular disease is a possibility, but a good colon exam, including a screening scope can be diagnostic.   If your colonoscopy is normal, than you can look at other possible causes.  As stated, a good exam and some testing will help you.
