A good exam should be thorough but not painful.

You can certainly do one on yourself, but have your physician do one during an annual exam too.

Rectal exam: It usually means a finger placed in the anus. It is usually part of a gyn exam. It is uncomfortable , but usually not more. I take it that is what you are asking.  If there is an intestinal problem no vaginal exam may be involved.
