Depends: If your disk herniation is acute (within the last few months), most spine experts suggest limiting heavy lifting until the pain subsides. When lifting is resumed, it should be gradual, starting with low resistance and more repetitions before adding to resistance. Check with your provider for specific guidelines in your personal situation.

Most commonly: it is a result of usual degenerative process. Where slowly the turgor of the disc is slowly lost, due to decrease in the amount of hydration in the disc. This results in disc space to lose height, then bulge out, which puts extra stress on the disc covering (annulus), which can slowly have cracks through it, and then the disc within will make its way out through the annulus, and herniate.

Aging, injury: Disc bulges are normal variants & not an issue that should cause a problem. This occurs with disc loading as well.  A disc herniation is an actual disruption of the disc's coating layers allowing disc material to move out of a normal position. This is a more frequent finding as we age as the disc degenerates which is accelerated by obesity & smoking. Overloading a disc's weight capacity=herniation.

Accepted Answer

Yes: A neuropathy is disease of a nerve root.  If a herniated disc presses on a nerve root, it will cause symptoms.  However, not all disc herniations cause neuropathy.  AND, not all symptoms of neuropathy are caused by disc herniations.  Often, symptoms will resolve with supportive care.

Spinal issues: stenosis is simply another word for "narrowing" of the space. So it's possible that a herniation (something going into a space it's not supposed to) is causing the narrowing. However herniation is not always the permanent disk problem people always think and this may be a different disc level. I'm not aware of the imaging obviously. Make sure to follow up with your physician to go over the imaging

Radiculopathy: Irritation to certain nerves (commonly the s1 nerve roots) from a herniated disc can manifest as pain radiating to the sole of the foot.

Herniated Disc: Yes, 
pelvic rotation can cause an asymmetric load on the iliolumbar ligament and effect the pressure on the disc.

Piriformis/sciatica: Piriformis syndrome is rare, usually secondary to trauma, and can be distinguished on physical exam by a competent spine specialist. In difficult clinical cases, MRI or other diagnostic testing such as emg/ncv may help pinpoint the source of nerve compression.

i have a herniated disc in my  lb. it causes severe pain when i stand up or sit. painful when walking but not bad when lying down. is that positive?

Sure: Sitting worse than standing. Lying down best. That is usually what happens with a herniated lower back disc.
