?cluster headaches: Could be chronic daily headaches, which may or may not be associated with pain medication.  However, some of the description suggests a cluster headache pattern, and may well be the answer, if this is awakening out of sleep at night.  Tramadol is not the answer, as you need a preventative agent in the migraine class.  See a headache specialist, and get under control.
