Vertical eye turn: An intermittent vertical eye turn in a child may represent a benign condition such as a congenital 4th nerve palsy.  This can also be seen in adults. Closed head trauma may also be a cause and be in the patient's history.  Although a vertical heterophoria may be longstanding and benign, it should always be evaluated by a qualified eye physician to rule out any underlying neurological conditions.

Strabismus: Strabismus is treated only if the person sees double most of the time.  The first treatment is prism glasses-- the images are aligned to be seen as one.  Then, if this does not work, and the deviation is stable, surgery to line up the eyes is considered.

First step is an eye exam to determine the cause and duration of the problem. Has it been this way since childhood? There are generally successful eye muscle operations to correct strabismus if the appearance is bothersome to the patient, and other neurologic causes have been ruled out.
