Itching to see MD?: Your vaginal itching, unless caused by a soap or contact chemical should be evaluated for a vaginal infection and Rx by a MD. A yeast caused vulvo- vaginitis is the most likely culprit.

Diflucan (fluconazole) takes days: Diflucan (fluconazole) takes a few days to treat a more stubborn yeast infection.  Itching indicates that the yeast is deep enough to create enough inflammation to trigger nerve endings.  So if after a week, the itching does not go away, i usually recommend using an OTC antifungal cream externally to finish the job.  If that does not help, see your GYN.

is a intense itchy vagina during first trimester on and off a sign of early cholestasis (icp)? rest of my skin very mildy itches but think cause of dryness. i had swab at 4 weeks it was negative but now intense vaginal itch is back?

Cholestasis : Cholestasis typically causes total body itching. It most often occurs in the 3rd trimester. There are many other common causes of vaginal itching. Schedule a visit with your Obgyn for an exam and testing.

Probably not....: ...Related.  Itching usually from an irritant such as soap etc.   Urine likely concentrated due to inadequate fluid intake.   However dark urine can mean kidney problems and should be monitored.   You may use Hydrocortisone cream for vaginal itching.  If this does not clear get checked for yeast and other infections.

Wonder about Yeast: Yeast can present with clumpy, white vaginal discharge to a watery, white discharge. Sometimes a discharge is not apparent. There is no odor. It can cause swelling and redness of vulva along with itching and burning pain. It may lead to pain with urination or intercourse. See doctor if not certain.

Yeast infection?: Yeast infection is the most likely explanation. Contrary to popular opinion, yeast infections usually cause little discharge and do not cause abnormal odor. See a doctor. Or you could try an over the counter yeast medication, active ingredient ending with "azole" (miconazole, clotrimazole, etc). But for sure get professional care if it doesn't help within a week.

Candidiasis: It could be due to vaginal candidiasis.
Candida albicans is a fungus that naturally lives on our skin and mucosae (mouth, genitals, etc). When the vaginal environment is disrupted the Candida multiplies and causes itching and swelling. 
It needs to be first evaluated by a gynecologist to be treated since there's  other possible causes

Yes: Vaginal itching without discharge can indicate infections like yeast or stis like herpes. It can indicate an allergic reaction to household products like certain soaps and laundry detergents. There are also some skin conditions liked lichen sclerosis which can cause vaginal itching. Hormone deficiencies from menopause or breast feeding can also cause itching.
