Rashes : Rashes are difficult to diagnose without actually seeing them.  With everything that you've tried, i think at this point it's time to see a pediatric skin specialist (dermatologist).  Call you doctor and ask for a suggestion of someone they know and like in your area.  Good luck!

Depends: It really depends on the condition being treated.  In general terms, cortisone injections relieve inflammation and pain.  Other modalities that can sometimes achieve this are: ice, oral medications such as Ibuprofen or steroids, rest, and splinting.  Physical or occupational therapy can also sometimes be helpful for musculoskeletal conditions and there is always surgery too.

Very few: The risks are small. Rarely this is a brief increase in inflammation which usually resolves quickly with the application of ice. Infection risks are extremely small.

Supression: Adrenal gland produces cortisol. External sources of cortisol(cortisone) will suppress the normal adrenal function.

Sometimes: In properly indicated cases, cortisone injections may fix shoulder pain.  Its usually helpful for pain from arthritis, and bursitis in the setting of an intact rotator cuff. Its not very helpful for rotator cuff tears, or labral tears.
