Should pain : More information is needed this can be several things. . Something lung problems cause shoulder pain if you have pain when you touch the shoulder deeply it would be more likely a shoulder problem. See your doctor

In a young man: there are several potential diagnoses that could present this way including collapsed lung, walking pneumonia, pulmonary embolus and a host of orthopedic issues. You would benefit from a thorough history, physical exam and probably a chest X-ray. Go get checked out now.

Hemoptysis?: If this came from a cough, it's called hemoptysis. If from vomiting, called hematemesis.  You should see a doctor.

The muscles that surround your shoulder blade can get strained or overworked either due to a shoulder injury (rotator cuff) or from certain overuse activities. Periscapular pain can be treated by massage to that area, heat can help, physical therapy to rehab the shoulder and also address the periscapular muscles and topical or oral anti inflammatory medications

If symptoms become more troublesome, see your physician or a neurologist for evaluation.

: You can get a sharp pain at the shoulder tip (which is the top most outer part of the shoulder) when your stomach is full enough to put pressure on your diaphragm. Probably when you drink something cold you drink enough to fill your stomach and put pressure on the diaphragm.

See doctor soon: If this pain occurs frequently you should have a complete evaluation not just and ekg and xray.  The pain may be coming from the lungs or the blood vessels from the heart.

Mgt/ workup: From your description the pain most likely is muscular and taking Motrin may help. If there is any difficulty breathing, please go to the ED for evaluation. 
Avoid muscular strain and using a heating pad are other measures which can provide relief. If the pain persists, worsens or is associated with any other symptoms, please see your PCP.
