Toxic goiter: Toxic implies hyper functioning. Uni nodular is one solitary nodule. This can be inflammatory due to autoimmune thyroiditis. This would relate to your itp which is also autoimmune. You are at risk of developing other autoimmune disorders in addition to thyroiditis and itp. These include colitis, arthritis, lupus, celiac, allopecia, vitiligo, psoriasis, etc...

Depends on agent: If using radioiodine, typically the tracer is given on day 1, then an uptake obtained at 4 and 24 hours later. If using pertechnetate, the uptake is done on the same day, and the scan and uptake together take about an hour or so.

Possible hypothyroid: Thyroid uptake depends on thyroid iodine trapping which is proportional to the rate at which the thyroid is making hormone. The total amount of iodine in the diet is also a major factor. If you use iodized salt and live by the seashore  your body will have more iodine available to dilute the test dose; thus giving a lower uptake value.High sensitivity TSH is a much more reliable indicator.

Yes: Capsules of 131i are given in small doses to see how much and where in the thyroid iodine can be picked up. Larger doses are used to treat hyperthyroidism due to graves' disease and toxic (multi)nodular goiters. Very large doses are used to treat thyroid cancers that pick up radioiodine. Talk to your doctor.

Tsh: Was your TSH low? If not, a thyroid uptake and scan might not necessary. How big were the nodules?  If they are bigger than 1 cm, an ultrasound guided biopsy is a better way to go than an uptake and scan. The scan can show size, shape and location of the gland which you already know from the ultrasound, and the uptake show function of the gland which you know from the labs.

Needs FNA: Typically you will undergo fna first. Correlation with ultrasonography is important as well as other lab tests including antibody testing.

why do the nodules of a multinodular goiter not show up on a i-123 thyroid uptake scan?

They are "cold": In this case, the nodules are non-functioning, so they do not take up the nuclear tracer.
