Many : Many children suck their thumb or fingers until age 2 to 4 years old. If they continue sucking  (to include a pacifier) once their permanent teeth come in, it can cause problems with how their teeth align.  If a child sucks her thumb really hard it can also impact her baby teeth. Some : Some thumb habits are more harmfull than others. It depends on frequency, duration and force.  Your 4 year old does not have any permanent teeth yet, but his habit can narrow his palate which will result in a malocclusion of both his baby teeth and his permanent teeth.

THUMB SUCKING: Sometimes breaking the habit of thumb sucking can be a challenge.The thumb can be infected yet they continue the bad habit. Getting busy and ignoring the habit is one way to try. T think this is the one that works. Putting hot sauce or tying the finger are not effective.

Slow extinction: You really can't "make" them stop, but you can start by incentivizing them to stop. Painting fingernails for instance, so they don't want to destroy the pretty image. Or working with them to go perhaps a morning without sucking, and if they do they get a sticker. Get enough stickers and they get a small prize. Usually the social group will get rid of it with peer pressure.
