High B12 and folate (folic acid): Levels are caused by their accumulation and non use in the body. I have seen it related to vitramin b1 deficiency. Worth trying.

Refractory sprue or: ...maybe some gluten is slipping into your diet. If your diagnosis of celiac sprue is a certainty,  then checking your tissue transglutaminase antibody now will indicate how successful you may be at gluten avoidance.

i have celiac disease and feel fatigue often. i have noticed iron supplement helps. can i take regularly iron + vitc supplement? signs of excess iron?

Celiac disease: You can take it, but should really be checked to see if you have any iron deficiency anemia issues.  Bc if you don't then there is no need to take the iron, which can cause GI side effects.

How high?: Celiac disease is not a usual cause of isolated lipase elevation. It would have helped to know the actual value along with the reference range for the lab that did the test.

See a hematologist: If your iron level is high, you need to be bled periodically to prevent damages to your organs particularly your liver. This is a continuing process and not a one-shot deal.  As for your thyroid, you need another test to confirm whether your thyroid replacement is adequate. Since natural thyroid extract is not standardized, it is hard to regulate your thyroid with it (used about 50 yrs ago).

Ferritin of 5 = low: That is a very low ferritin, so this is most likely iron deficiency, though you may not yet have anemia. With this ferritin it is likely you eventually will get anemia. Iron supplementation is generally safe but also look into iron containing foods to increase intake.

With a ferritin of 5: ...Probably yes. If it's 5 on the same scale as most of us use, that shows low iron even if there is no anemia.

No: In fact people taking PPI class of drug for GERD often end up with low serum B12 level which may or may not be clinically important.

Depends.: Each lab has its own reference range for out of range celiac panels.  I would refer to those that come along with the lab report.  However, you don't have to have positive tests to be gluten-sensitive.  Why not just cut gluten out of the diet for 8 weeks and keep a journal?  This is cheaper.  If you do go gluten-free, make sure you "play all out' and not cheat.  Otherwise, no sense in self-torture.

does the tissue transglutaminase iga level need to be above or below 0 - 10 to signify celiac disease?

Accepted Answer

Confusing post: If the level is above 10, it is considered positive for CD, but additional studies, often an intestinal biopsy are required for conclusive labeling. If a person does not make IgA normally, the test could be falsely negative, a TTG-IgG would be needed for the screening test on such a person. In either case, some consumption of gluten for 3 weeks prior to testing is required to assure accuracy.
