Varicocele : It could be due to an abnormal collection of veins in the scrotum. An ultrasound should be able to tell if that is the cause. The other possibility is a hydrocele, which is a fluid filled cyst in the scrotum. A referral to a Urologist may help with the diagnosis and treatment.

Possible hydrocele: Most common cause for scrotal swelling in toddler boy is hydrocele which is a benign collection of fluid around testis. Commonly associated with incomplete testicular descent. Can also be caused by an inguinal hernia. At this age rarely from testicular torsion associated pain &amp; redness, &amp; an emergency requiring immediate treatment. V. rarely from testis tumor. Consult  pediatric urologist/surgeon.

Dilated veins: Dilated veins in the left side of the scrotum are called a varicocele and are caused by backflow of blood and poor function of valves in the veins. Worrisome: This is really worrisome to me.  Continued testicular pain, even if it is intermittent can be a sign of a " twist " in the testicle.  This can happen in boys and it can cut off the blood supply causing pain.  Lack of urination is also concerning.   I would bring him to an Emergency Room where he can be evaluated by the ER staff, preferably one with a Urologist on staff.   Go now.

WBCs: If you have white blood cells in your urine/semen, then there is an infection somewhere.  You need to see a doctor for antibiotics.  Best of luck!

Lower abdominal pain: is usually related to a bladder or large intestine problem, such as constipation.  It is normal to have prominent veins on the surface of the scrotum when in a relaxed state, but if there is a cluster of veins, it could be a varicocele which occurs in about 15% of men and is usually L sided  but usually is painless or dull ache instead of abdominal pain. I recommend you get an evaluation.
