No: The groin and the testicle are not the same and there may be very specific issues that require very specific and urgent treatment.  The testicle can become twisted or an appendage off the testicle can become twisted.  One may have a hernia, hydrocoele, or other issue.  If you have recurring pain with running in the groin or genitals, seek immediate evaluaiton.

Most common cause of  pain in men older than 18 and if sexually active then likely due to sexually transmitted disease such as gonorrhea or chlamydia.

Go to the ER now...: There are many reasons for testicular pain. Bottom line, if you have testicular pain...Pain in the testicle...You should go to the nearest er immediately to have special imaging to rule out a torsion. Testicular torsion is when the vessels to the testicle twist and the blood supply is lost. If not corrected immediately you could lose the testicle. You may experience belly pain, especially in kids.

Unlikely: It is his testicle and unless you purposely injured it is not likely to be your fault.  Normal intercourse does not cause testicular pain.  However, prolonged erection causes vascular congestion in the groin in general, including in the testes and may leave an ache in the testes.  Other causes of episodic testicular pain may be varicose veins and inflammation.

In older men, it is often due to urinary tract pathogens such as e. Coli and proteus. If you have significant testicular pain, please get evaluated. A scrotal ultrasound can determine if scrotal pain is due to infection or torsion.
