A : A monthly self-exam of the testicles is recommended. Some men do this after a warm bath or shower, so the scrotal muscles are more relaxed and the skin is softer. The testicles should first be visually examined to look for bumps or swelling. Then they should be palpated by holding the scrotum and testicle in the hand. Gently roll the testicle back and forth between the thumb and forefinger. Abnormal findings in the testicles that warrant further evaluation are a painless swelling or lump, any change in how the testicle feels, any change in size, an acute build-up of fluid in the scrotum, or pain in the testicle. Further testing may include an ultrasound, a painless procedure during which high energy sound waves are bounced off the testicular tissue.

See your doctor: Usually, cancer does not present as pain, but infection or inflammation can.

Either way, please see your doctor and get a complete examination, to possibly include urinary analysis and if appropriate, testicular ultrasound.

Annual exam: Your annual exam will tell you if there is any suspicion of testicular mass/testicular cancer. Exam alone will not be able to tell you a definite answer whether you have a cancer or not. A series of tests that include sonogram of the testes, blood work- such as Alpha feto protein , b-hcg, other imaging ct/ pet scan  etc have to be done--then ultimately orchiectomy will give you a definite answer

When surgery?: Current studies pretty conclusively show that if the undescended testicle is brought down before first birthday, there is not increased risk of cancer. That doesn't mean no risk - just not any more than general population.  You are at prime age for testicular cancer, so follow doctor's instructions and do monthly exams yourself. If you feel lump, I'd be more concerned than size difference .
