Many possibilities: Your pain could be as a result of many different things. A thorough history and physical exam and possibly some laboratory and radiology exams are needed to determine the exact cause. Causes of pain may include: trauma (fracture, sprain, arthritis), infection, metabolic conditions (gout), musculoskeletal or biomechanical abnormality and neurologic conditions (neuropathy).

Coccyx bruise or fx: Most likely you bruised or fractured your coccyx, or tail bone.  It is rarely a surgical problem, but it can take weeks or months to resolve.  Generally, we recommend warm water sitz baths and over-the-counter analgesics like acetaminophen, Ibuprofen or naproxen.  If it hurts to sit, buy a coccyx cushion.  If the pain is severe, see an orthopaedist.

Pilonidal cyst: It could be due to a pilonidal cyst, which is a cyst containing hair or skin debris that forms around your tailbone area, which can get infected and very tender when sitting down. You should see a doctor for evaluation and treatment.

Injections and Meds: Coccydynia is a chronic pain around your tailbone coming from the abnormal anatomic relationships and strain on the supporting ligaments. There can also be some regional nerve damage. Frequently, we use injections around the coccyx to calm down the pain, along with appropriate medication management. Be advised that this may not "go away", needing occasion treatment to "control" the symptoms.

Pubococcigeal muscle: I don't thing so. The pubocoggigeal muscle has a role in the closing and opening the urine flow and some role during the kegel exercises that help to preserve the muscles of the pelvic floor somehow related to sexuality. It is unlikely coccyx ( tail bone)pain or fractures have a direct effect in the female genital tract. Do kegels exercises. If you experience problems consult your dr.

Needs evaluation...: It is difficult to tell without more information or a check up. Possibilities could be infection in his testicle or a UTI, severe constipation, low back problems which could reflect  to his pelvic area. He needs to see his doctor for a formal exam and diagnosis.
