Anatomical assymetry: This is hard because something sounds very wrong and i would want your son evaluated urgently.  Here would be my rule:  if he is having any difficulty breathing, is lethargic or has a fever then get him to the er.  If he is comfortable, able to communicate in full sentences and is his normal personality then you can wait to see the pediatrician tomorrow.

See above for causes of gynecomastia . Rarely  ingestion of significant amounts of soy products can also cause gynecomastia. Would follow up with  MD

See your doctor: Your symptoms may be signs of something significantly wrong.  One cannot tell without a complete history, physical and possible lab/imaging tests.  Follow with your doctor as soon as you can.  Things that can cause pains in chest and ribcage include heartburn, pneumonia, gallbladder problems, liver disease, kidney problems, heart attacks, or blood clots.

Doctor can evaluate: A person with aches at the chest, rib cage, or left shoulder can see his doctor to be evaluated. Pains can come from a heart problem (serious!), neck and shoulder muscles, the ribs and their joints, the lungs inside the chest, etc... If one feels his pain is coming from the heart, has a chest tightness, some difficulty breathing, or has past heart disease, he can go to the E.R. by ambulance.

You should get Physi: A Physical examination by a doctor is the only way to verify that your left side of chest is swollen. if it is really swollen then further testing starting with chest X-rays will be required to check your heart and lungs including the chest wall structures. Could be serious: If you have no history of swelling (edema) and especially if you have new onset chest pain you need to go to the ER right now or call 911.  These symptoms could be due to heart disease. There are other causes, but it is crucial for you to be certain that that is not the reason, and risk serious consequences by delaying treatment.

Costochondritis....: Costochondritis is caused by inflammation of the cartilage of the chest wall. Anti-inflammatory meds, like ibuprofen, are given to relieve the pain of the inflammation. Physical therapy modalities can also be helpful so have your doctor send you to physical therapy for evaluation and treatment.

Need more info: It would be helpful to know if you are over weight or very slim. Your age is also important. It could all be anatomically normal but nevertheless it should be investigated by your family physician.
