RA: Today there are many treatment options for RA. Many of the drugs do lower the immune response and may predispose to infections such as TB. Nevertheless, rheumatologists are able to mitigate these side effects even in patients with latent TB. We sometimes recommend anti-inflammatory drugs such as ibuprofen for flares. Other Rx: Even though tnfs did not help, there are other biologic agents which have shown good results. One drug is Kineret (anakinra) which works on il-1 ( interleukin-1) instead of tumor necrosis factor (tnf). Be sure to let your rheumatologist know that you are still in pain and that you cannot taper off the steroids.

Continue followup: As you have mentioned, it sounds like for some reason you are retaining fluid.  There are multiple potential causes including cardiac, renal causes.  Need to find underlying cause and correct diagnosis before appropriate treatment begun. Numbness in palm possibly carpal tunnel syndrome, a condition that can be aggravated by fluid retention.  Don’t hesitate to see specialist if not making progress.

Not much: While we often give "antiinflammatories" like candy to control pain and inflammation, the medications have only a very small affect on swelling. The majority of the benefits of the antiinflammatories are the pain-relieving aspects. They also have almost no affect on swelling inside the knee from osteoarthritis. Most of the fluid occurs from mechanical wear or bleeding within the joint.
