Nothing to Hum About: The "hilum" is in the middle of your chest and there ain't supposed to be no masses there!The mediastinum is in the same general area but is bigger and contains the hilum and a bunch of other stuff.Since they said just a hilar mass that means it's a part of the large vessels and lung  tissues near the heart and not any of the other stuff in that area. You need to get this cleared up with your doc

Yes, regional metast: If this is a case of lung Cancer, then lymph node spread to the hilar areas and mediastinum means regional spread of cancer. This is still treatable but patient needs to go see a Cancer specialist(Medical Oncologist)  at a recognised Cancer Center or  Medical center where expert specialists who treat Lung Cancer would be more easily available.

i am going through testing for possible hodgkins lymphoma. my ct scan reported anterior mediastinal hazy density. should this be biopsied? does this mean a mass?

Possible mass: You may need a PET thoracic scan and bio marker panels to determine the cause. Please follow up with your Pulmonologist.

Needs Biopsy: Is there a known primary cancer? If not, you will need a biopsy of one of the nodes. Your doctor may suggest a mediastinoscopy and biopsy to get a tissue specimen. There are other causes of mediastinal node enlargement such as infection and sarcoidosis  so it is important to make a definite diagnosis.

A few possibilities: Several things other than cancer can cause such abnormalities. Various infections and sarcoidosis come to mind. There's no way for a forum doctor to say anything more. The doctor(s) treating you or who did the bronchoscopy are the best to ask.

Sarcoidosis : Sarcoidosis is a diagnosis of exclusion that requires biopsy/histologic evaluation and exclusion of other diseases including lymphoma.  A 5.7 cm posterior mediastinal mass warrants evaluation by a thoracic surgeon.
