: Your neck muscle tightness is probably pinching the scapular nerve as it exits from the neck &amp; travels down to the shoulder blade. Treatment would be measures to relax the neck muscles with intermittent hot water compresses for 20 minutes. Muscle relaxants may help. See your MD as needed. Neck xrays,MRI, physical therapy may be ordered if deemed advisable.

Could be: Anxiety can cause muscle tension.   Sounds like you may have a muscle spasm near your shoulder blade.  It usually resolves on its own, or you could:  1) get it massaged, 2) Lie down on floor on 2 tennis balls, one on either side of your back and each symmetrically next to the shoulder blades, and lie on them for a few minutes or until the spasm relaxes, 3) try heating gel topically.

Muscle spasm: Spasm of muscles in one are often causes other muscles to tense up. Hot compresses may help and you may take over the counter pain medication.

Yes: Anything that causes strain or spasm of the muscles can cause pain in those muscles or muscle groups.  The area your talking about is probably the second most common area of the body to experience this, the first being lower back.

Neck Pain: Neck pain can radate and cause headache . It can also cause muscular pain in both shoulders and in between scapulae.

Musculoskeletal: A late teenager with neck ; upper back pain bilaterally likely has a musculoskeletal problem.  Indeed vitamin d deficiency is often concomitant with chronic pain syndromes.  My own theory is that the deficiency is possibly caused by household reclusiveness and sun avoidance due to a sedentary lifestyle.  See an osteopath to find out what causes the chronic pain.  Evaluation by physical therapist.

Lymphadenopathy: You need an exam hard lymph nodes that are painful and immovable  are worrisome so I would let your practitioner touch it and see if further work up is needed

Stress?: Several of the more common reasons for muscle spasm and tightness in the neck and shoulder area include: Stress, tension or anxiety causing you to contract the muscles unconsciously. Sleeping in an uncomfortable position. Injury to the neck. See your physician for a proper diagnosis and treatment.
