Multiple reasons: It can be multiple reasons ,first of all can be pregnancy,and then hormonal imbalance or endometrial polyp /cervical polyp /pelvic infection .Please see your obgyn for evaluation

Breakthrough bleed: Sounds like breakthrough bleeding which occurs when you have bleeding at unscheduled times while on the pill.   Often things get back to normal the next cycle. If not then schedule a visit to discuss changing brands  or changing birth control methods. This is very common. Other than being annoying it is not dangerous.

Irregular ovulation: could be a cause,  or medication effects if on contraceptive hormones.  See your doctor if this continues.

Caution: No need for worry, but you should be cautious about anemia if your flow is heavy. I suspect your clinician scheduled a followup 2-week appointment after you were given antibiotics. Keep that appointment, but you should be ok until then. Hope that helps.

No:  DUB: It is called dysfunctional uterine bleeding related to abnormal changes in hormone levels. The spotting is likely caused by some unexpected variations in your body's hormones. It would be more unusual if you are taking birth control pills as these tend to regulate hormones. It will probably normalize next cycle. If bleeding persists longer than 2 weeks or gets heavier, contact your doctor

Spotting with BCP's : without knowing the pill   it sounds like the hormone levels need to be rebalanced.   Given your age you may need a higher dose of estrogen to control spotting.   Discuss this with your doctor  -  Generally BCP's help with spotting or intermenstrual bleeding and don't cause it.    Good luck

Some : Some patients bleed and spot for several months after putting mirena (levonorgestrel). Usually it stops by end of 6 months. So if no other symptoms be patient. If bleeding is heavy and is associated with pain please see a physician.
