Uterus: Anteverted and retroverted uterus is not about the shape of the uterus, it is about the position of it. Possibly the pregnancies stretched the ligaments supporting it and it's changed its position. Both positions are well within the normal range, though.

Tipped uterus: Usually no effect, rarely the topped uterus may be stuck in the pelvis with a lot of pain.

Not really: a retroverted uterus can sometimes cause some lower back pain during pregancy- just the way your anatomy is- not dangerous.

Yes: As has been said before "nature always finds a way".  It is important to followup with a doctor soon after discovering a pregnancy so the appropriate care can be given and complications from the mesh can be alleviated.

Before pregnancy: The endometrial lining of your uterus actually thickens in the second half of your menstrual cycle (the secretory phase) after ovulation, in preparation for pregnancy, as a result of an increase in the hormone progesterone.  If you do not get pregnant, your Progesterone level drops, and your uterine lining sheds and is expelled during menses (your period).  The cycle then starts over.

Palpable means: That it can be felt. So, the uterus is large enough that physician can feel it when examining you.

Grapefruit sized...: At 3 months, the uterus is the size of a grapefruit.  At 4 months its the size of a volleyball.  Best wishes!

What kind of hole: You have 3 openings in the uterus (2 from the Fallopian tubes feeding into the uterus and one below at the level of the cervix to allow sperm to come in and your periods to exit). These are natural orifices. If you had additional defects in the uterus, you should see your GYN to determine the location and implications on your pregnancy
