More info needed: From your question, you must have already seen an ENT surgeon.  I would direct your question to this doctor.  Additionally, how your infant is feeding would be valuable information and whether you need special nipples for feeding.
Good luck--a lot depends on how the cleft is structured.

I've never seen it: I've dealt with the issue for three decades & never seen it as a primary cause. If the kid has a sleep disorder related to sleep apnea/tonsil size i could see it causing chronic health problems, but not changing the emergence of speech & language skills  in a kid that was ready.

Growth plate irritat: There is a growth plate in the location cited that sometimes becomes irritated yielding pain. Anti-inflammatories like ibuprophen may help some  while a cool compress can provide some pain relief. Reduced physical activities may be required for a while. This is confirmed more by exam than x-ray.

Common autism trait: One of the hallmarks of autism is the impaired ability to recognize or interact to social cues in the usual way.  No one really knows why this happens, but is quite common in autistic children.  If possible, a hearing screen can be performed to make sure hearling loss is not a contributing factor.

The underlying issues need to be explored but a form of communication using a variety of accessories is possible. Your physician should know someone with these skills.
