I think you: Are reading and MRI report.  The acromio-clavicular joint is in the shoulder between the acromion process of the scapula (shoulder blade bone) and the clavicle (collar bone).  An effusion is fluid in a joint.  Synovium is a lining of a joint.  Hypertrophy means that it has become large.  In translation, it means you have a slightly damaged shoulder.  Follow your doctor's advice.

Significance: The pericardial sac encapsulates the heart. If an effusion as small, it is not enough to interfere with the mechanical work of the heart, thus clinically insignificant. If it is large, it can impinge on the walls of the heart, forcing them in and limiting the volume of blood they can pump. The significance determines how aggressive they would be to remove the fluid with a needle or operation.

Talk to doctor who ordered echo b/c implications depend upon you.

Fluid collection: There is a lining that covers your lung and a lining that covers the chest wall.  There is normally a small amount of fluid in between the two.  However, when more fluid collects then it can be seen on a cxr.  Causes are many.   A ct chest can help further help evaluate the effusion and the lining of the lung.  Sometimes sampling the fluid with an ultrasound guided thoracentesis is neccessary.
