Some thoughts: There are a few things this could be, like infections, mechanical problems associated with the circulation of the testis, or tumors involving the testis or other structures in the scrotum.  If this is a persistent problem, i would strongly recommend that your husband see his doctor or a urologist about this issue.

Torsion: Depending on the intensity and progression of the pain. Could be epididymitis, or could be a torsion. 1st will be rather steady pain (see dr), 2nd will be progressively intense/severe pain (go to er), as it is due to loss of bloodflow to testicle. Need ultrasound test to confirm.

This needs eval: There is really no way you can figure out what this is without an examination. It could be painful and swollen due to trauma, fluid build up (hydrocele), inflammation (orchitis), or many other causes. If the pain was only mild, you could take an OTC pain reliever and wear an athletic supporter for 2-3 days, but if pain is sudden/severe you need evaluation now. Stay well:)

Potentially there: are a number of causes. Most concerning would be torsion. Potentially you could have an inguinal hernia that is sliding down while exercising. You might be causing trauma to the testicle from banging against your thigh during play. Ultimately it will be impossible to diagnose you over the internet. Make an appointment for an examination by a health care professional.

: Some times a small epidydimal cst can cause this if the testicle is in in a position where it gets irritated. Just change the position of your testicle. Wear a good support underwear. If you feel a lump see your Urologist.

Intermittent torsion: Is a possible explanation, could be confirmed if you twist testicle counterclockwise till painful. Probably intermittent torsion if twisting exactly reproduces pain. Small epididymal cyst might also cause this symptom. Might also be vascular congestion in spermatic vein or varicocele. Suggest you consult urologist & ?Get scrotal us. Good luck.

Testicular pain: A painful testicle can be a symptom of swollen veins inside the scrotum (varicocele).  This can result in one testicle being larger than the next and painful.  A tumor is also possible as is epididymitis.  You should see a urologist and have an examination and possibly an ultrasound of your testicles.
