Be seen: If you are having chest pain that is bad enough that you can still feel it after taking 8 mg of hydromorphone (which is eight times as strong as morphine) I would definitely be seen to rule out a cardiac event,perforated ulcer, or other serious condition.

Chest wall pain: Your symptoms are likely coming from your chest wall. Heart pain is usually in the center of the chest and is more often described with a clenched fist or with words like heaviness/tightness/pressure/burning or "like an elephant sitting on my chest". Heart pain is constant, not pulsating, and not located in one small area you can point to with one finger. It is not sharp, stabbing or needle like.

Maybe the pleura?: Should get doctor's exam. The pleura is the lining on the inside of the chest and on the outside of lungs. It does have some sensation, so might be the source of the sharp brief stitch-in-the-side feeling that is happening. A primary care doctor can carefully listen to the lungs, and also talk with the patient to see if there are other signs or symptoms of a problem. Sometimes chest x-ray is done.

Heart attack sx can include:   chest pain or tightness, shortness of breath, increased sweating pain or numbness radiating down the arm (more often left than right) and nausea. There could be pain in the upper back, head, jaw or teeth. One fourth of people have heart attacks without chest pain.
