Studies have shown: That the differential diagnosis of heartburn symptoms is quite broad, but according to Dr. Harmon and others in Therapeutic Advances in Gastroenterology,  there are five major causes:
Medications, functional dyspepsia, GERD, peptic ulcer disease, and cancer.

due to my severe gerd severe heartburn when i take antibiotic pills despite being on ppis. do all antibiotics come in injectibles as well? tyvm

GERD: Many antibiotics do come in injectable forms.  However, if your heartburn is that severe, I would encourage you to speak with your provider.  If not already done, you would benefit from an upper endoscopy to see what's going on in the upper GI tract.  You might also need twice daily PPI for a period of time.  You should have this checked.

Timing : How are you taking the omeprazole? It should be taken on an empty stomach with nothing to eat or drink for at least 30 minutes after. If you aren't taking it this way that can reduce to the effectiveness. Otherwise, you may need to see your doctor or a gastroenterologist.

Stomach issues: You may have GERD, but I suspect stomach pathology more. Gastritis(inflammation of the stomach lining) or gastric ulceration are possibilities in view of your poor appetite. The gastroscopy will be very helpful in ruling these stomach disorders in or out. In the meantime avoid alcohol, caffeine, nicotine, spicy and greasy foods. Your gastroenterologist will guide and advise you.

GERD: Symptoms from GERD often are alleviated rather quickly (several days) with medications like Nexium, (esomeprazole) Prilosec among others that are available over the counter now. If symptoms persist then an endoscopy should be done.

Not aware: I  not aware of one. Gerd could be a complex disease and specific to every pt.  The best thing you can do is be evaluated so you get personalized, specific treatment.

You should set up a : schedule with a Gastroenterologist as to how often to monitor your esophagus. Also, discus surgical option for condition.

GERD: The common risk factors are eating large meals or lying down right after a meal, being overweight or obese, smoking, citrus fruits, and drinking certain beverages, such as alcohol, carbonated drinks, coffee, or tea. Treatment includes avoiding trigger factors, not eating at least 2 to 3 hours before lying down. losing weight and taking antireflux medications. Discuss NIssen funduplication with Doc

Problems : The most severe symptoms of GERD would be pneumonia from reflux going in to the lungs, or massive bleeding from severe esophageal ulceration. Over the longer term, a stricture of the esophagus or the development of esophageal cancer are also quite bad. Fortunately, these are all uncommon. Unfortunately, they are not predictable for an individual. Hope this helps!
